Travel Insurance Conditions Statement
I confirm receipt and reading the Document containing information about the insurance product. Cancellation costs, General Insurance Conditions, Reimbursement Costs (OWU/16/128491/2017/M) and Information about the purposes and principles of personal data processing by TU Europa SA/TU na Życie Europa SA before concluding the insurance contract, as well as their understanding and acceptance.
I declare that the product meets my needs and requirements and I apply for the conclusion of the insurance contract.
I authorize TU Europa SA to obtain from entities carrying out medical activities within the meaning of the regulations on medical activity, which provided me with health services, information on my health, as well as the reasons for death (excluding results of genetic tests) related to the determination of the right to the insurance benefit or the amount of this benefit to the extent resulting from generally applicable laws.
